The Home Delivered Meals (HDM) Coordinator is tasked with overseeing and managing the implementation of the Home Delivered Meals Program (HDM). Coordinates with St. Vincent MOW for efficient meal delivery to members. Responsible for ensuring proper delivery of meals from caterer to Homebound members on a daily basis, responsible for data entry of new members’ information, update and delivery sheets. Schedules and conducts in-home assessments for HDM participants to determine eligibility for HDM program as well as for referral for other services.
